Case management serves as a means of linking seriously mentally ill adults and seriously emotionally disturbed children and adolescents to necessary community support systems and services. The goal is to promote recovery and successful community living, and to reduce the need for more intensive services. The primary function of the case manager is to assess community service needs and develop strategies to better meet those needs.

These services are available Monday through Thursday from 8:00 a.m. to 5:00 p.m. and Friday from 8:00 a.m. to 12:00 p.m. in Dallas, Perry, and Wilcox Counties.

Case Management Services are available to all consumers with Developmental Disabilities to help them function independently in the least restrictive environment.Cahaba Center, as the designated Case Management Agency for Dallas, Perry, and Wilcox Counties, performs activities which include but are not limited to serving as initial point of entry for individuals who are seeking services.  Assessments are conducted for eligibility determination and submission to the Regional Office for review and to enter persons on the State Waiting Lists. Other activities include but are not limited to: needs assessment, case planning, service arrangement, social support, reassessment and follow-up and monitoring. These services are provided in a variety of settings and involve interaction with individuals with Developmental Disabilities, their families, generic and specialized service providers, public/governmental agencies, and the general public. Services are based on a person-centered approach with case managers focusing on assisting individuals in making personal choices which would lead to a satisfying and normative community life. If an individual is not eligible for Developmental Disabilities Services, the case manager will assist in a more appropriate referral.
