Case Manager - Stepping Up Initiative / Assisted Outpatient Treatment

Job Description

Ensure program compliance, reporting, monitoring, and tracking performance.
Work with probate court.
Provide transportation services.
Assist with establishing MOA's, plan and conduct "crucial conversations" with community partners, facilitate treatment team meetings, and work directly with all outpatient program leadership.
Other duties as assigned.

Prerequisites

Requires a bachelor's degree in a Human Services field as well as a valid AL driver's license.
Experience working with individuals with serious mental illness and collaborating with the criminal justice system is preferred.
